A spokesperson with School District 51 admits the parent of one student on a crashed school bus was never notified her daughter was onboard.

Jeff Kirtland with District 51 says the miscommunication happened over two students with the same last name.

A mother of one of the students with the same last name already knew about the crash, so school official didn't call the other mother, mistaking the two for both being the mother of the same child.

Kirtland says District 51 will do more validating and cross-checking in the future to prevent this from happening again.
