DENVER (AP) -- Colorado child protection workers made significant efforts last year to identify more cases of child abuse and neglect, but a failure to accurately complete risk and safety assessments continues to affect how county departments handle maltreatment cases.

A report Tuesday from the Colorado Department of Human Services examined cases of child fatalities, near fatalities and incidents of abuse or neglect from 2012. Officials say the state's computer system makes it difficult to track cases from county to county.

According to the Denver Post, nine fatalities, two near fatalities and one incident of egregious abuse qualified for full fatality review in 2012.
