Elizabeth Costisick has been sentenced after she provided alcohol to a minor that got in a DUI crash and died.

GREELEY, Colo. (AP) -- A 31-year-old Johnstown woman who acknowledged providing alcohol to a minor who died in a DUI crash the same night has been sentenced.

The Greeley Tribune reports (http://bit.ly/13b14iZ) Elizabeth Costisick was sentenced Thursday to two years of probation. She also must wear an ankle monitor for six months, complete 80 hours of community service and attend a Mothers Against Drunk Driving impact panel.

Costisick, who worked at a Johnstown bar, was accused of serving 19-year-old Waylon Ehrlich, of Milliken, on April 14, 2012. The teen crashed his pickup later that night near Longmont.

As part of Costisick's plea deal, prosecutors dismissed three other counts of providing alcohol to a minor.
